How do I get these credits?

• Print off attached form or call into Careers T20 and collect form
• Fill out the form
• Photocopy your licence in colour! The front and back of your licence must be on the the same page not to separate sheets
• Then bring your  licence, completed form and the photo copy page to me in Careers T20 to verify and action your application which takes about 2 mins
